What You'll Do \n As a Transitions Hospice Licensed Social Worker, you'll carry a meaningful caseload and serve as a trusted advocate and counselor for patients and families throughout the hospice journey. Your responsibilities include:

* Conducting thorough, compassionate initial psychosocial, spiritual, and pre-bereavement assessments for patients, families, and caregivers \n * Providing ongoing emotional support and counseling as a consistent presence throughout the care journey \n * Identifying and connecting families with community resources — including financial assistance — to meet practical and emotional needs \n * Guiding families through Advanced Directives with clarity and sensitivity \n * Assisting with facility placement for patients when appropriate \n * Maintaining accurate, timely clinical documentation at the patient's residence and in agency records \n * Actively contributing to interdisciplinary team and IDG meetings to shape holistic care plans \n * Participating in on-call duties on evenings and weekends as needed \n * Managing caseload while Assuring compliance with local, state, and federal laws, Medicare regulations, and established personnel policies and procedures \n\n Benefits We Offer

* Master's degree in Social Work. Master's degree from school accredited by the Council on Social Work Education required in states as specified. \n * Licensure and/or certification as required by the State. \n * Minimum of 2+ years social service experience in a health care setting or social services agency. \n * Experience in hospice, palliative care, or a related healthcare setting preferred \n * Deep comfort with grief, loss, and end-of-life conversations \n * Strong knowledge of community resources and care coordination \n * Excellent documentation skills and attention to compliance \n * Self-directed with the ability to work with little supervision \n * Valid driver's license and willingness to travel locally for patient visits \n Transitions Hospice - Illinois
