Job Overview

This position involves the assessment and provision of physical therapy services in alignment with the organization's standards, as well as the laws and regulations applicable in Maryland, the District of Columbia, Virginia, and other relevant guidelines.

• Assist in achieving departmental goals while adhering to policies, procedures, quality benchmarks, and safety protocols. Ensure compliance with governmental and accreditation standards.
• Exhibit foundational leadership abilities as a Case Manager and fulfill additional responsibilities as necessary.
• Act as the primary preceptor and mentor for newly hired nurses and home health aides, overseeing their field training. Collaborate with field trainers to ensure new hires receive the necessary learning experiences to achieve clinical competency. Recommend and provide further education based on identified clinical skill needs.
• Conduct Joint Observation Visits for skill validation and participate in mock surveys as required.
• Demonstrate a comprehensive understanding of Oasis documentation and ensure adherence to regulatory documentation requirements, including care plan development and goal setting, while guiding clinicians in proficient documentation practices.
• Develop and implement effective physical therapy treatment plans in collaboration with patients, their families or caregivers, physicians, and other healthcare team members, adjusting plans as necessary. Evaluations will include interviews, observations, palpation, and specialized assessments such as goniometry, muscle strength testing, and sensory testing, utilizing appropriate tools.
• Independently execute and document care plans for patients with varying complexities to ensure continuity of care. Documented interventions should align with the patient's condition and reflect the standard of care, incorporating agency and community resources as needed.
• Engage in service recovery efforts by addressing patient and customer concerns, coordinating with interdisciplinary teams, and implementing solutions that enhance patient satisfaction and operational excellence.
• Delegate cases to Physical Therapist Assistants (PTAs) as appropriate.
• Initiate and participate in regular case conferences involving all relevant disciplines, including patients and families in goal setting and care plan adjustments.
• Maintain effective relationships with other departments and engage in multidisciplinary quality and service improvement initiatives. Participate in meetings and committees, representing the department in community outreach activities. Pursue personal growth and skill enhancement through educational programs and affiliations.
• Conduct patient history and assessments to determine habilitation and rehabilitation needs, recommending adaptive equipment or home modifications to optimize functional abilities and safety. Identify primary patient issues and discharge planning needs, including referrals to community resources, while interpreting abnormal clinical data to foresee potential problems.
• Administer various procedures and modalities, including therapeutic exercises, functional mobility training, gait training, heat/cold applications, and ultrasound.
• Serve as an educator within the discipline, providing feedback and suggestions to enhance workflow and standard procedures. Collaborate with the Education and Operations Team to create and update an individualized clinical orientation program for new staff.
• Perform additional duties as assigned.

• Bachelor's degree in Physical Therapy from an accredited institution is required.

• 1-2 years of clinical experience is required.

• PT - A valid Physical Therapist License to practice in the District of Columbia, Maryland, or Virginia, depending on work location, is required upon hire.
• DL - A valid State Driver's License in good standing and access to a reliable vehicle for transportation is required upon hire.
• CPR - Certification in Cardiac Pulmonary Resuscitation (including BLS and NRP) for healthcare providers from the American Heart Association or American Red Cross is required within 90 days of hire.
• Additional unit or specialty certifications may vary by department.

• Understanding of home equipment, home care reimbursement, and regulations governing PTAs.
• Basic computer proficiency is preferred.
• Experience with smartphones.

The salary range for this position is between USD $89,065.00 and USD $162,801.00 per year.
