The Primary Care Provider is responsible for preventative care for the patients as their first point of contact and takes continued responsibility for providing the patient’s ongoing comprehensive care, focusing on their long-term health and well-being. The Primary Care Provider must be able to work in a team-oriented environment, be flexible to adapt to new technologies and protocols in a quickly evolving practice setting. They must have the ability to respond quickly and accurately to changes in condition or response to treatment and is responsible for providing outstanding patient service within the clinic and through various virtual communication channels, while maintaining a compassionate and welcome atmosphere. Primary Care Essential: Conduct thorough physical examinations and assessments for patients of all ages, including children ages 3 years and up. Perform chronic care management for patients with conditions such as hypertension, diabetes, and asthma. Administer annual wellness visits for adults and pediatrics, including vaccinations and preventive screenings. Order, interpret, and evaluate diagnostic tests to diagnose and assess patients' clinical problems and healthcare needs. Collaborate with physicians and other healthcare professionals to develop comprehensive patient care plans. Prescribe medications, recommend treatments, and provide written home-going instructions. Refer patients to specialists or other healthcare resources as needed. Educate patients and their families on health maintenance and preventive care. Promote patient engagement, health literacy, and shared decision-making. Participate in quality improvement initiatives and value-based care programs. Emphasize patient-centered care, continuity of care, and building strong patient-provider relationships. Essential Functions Ability to provide quality care meet patient volume goals, expectation of +18 daily on average. Preferred experience working as a Telehealth Clinician Lead clinic staff in behaviors, actions and attitude (e.g. X-Ray Technologists, Nurses, and Front Office Specialists) in delivering excellent patient care. Provide guidance as necessary to ensure quality professional services and excellent patient satisfaction. Discuss and review patients’ medical history, symptoms, allergies, and current medications to formulate accurate diagnoses and provide guidance to ensure quality professional services and patient satisfaction. Engage with clinical leadership to ensure strong patient care and adherence to company values. Ensure accurate completion of patient charts in a timely manner and forwarding charts as appropriate daily. Ability to work efficiently in a fast-paced autonomous environment. Dedication to evidence-based medicine and staying current with medical advancements. Implement clinical protocols as outlined by the CMO and Supervising Physicians. Operate within standard NP protocols. Analyze and interpret patients' histories, symptoms, and diagnostic information to develop appropriate diagnoses. Deliver excellent patient care with attention to patient-centric care, safety, cost, and accurate information. Ensure activities comply with department expectations and applicable Federal and State regulations, including HIPAA. Complete mandatory compliance training and pursue continued quality of care. Adhere to the Core Values of teamwork, communication, empowerment, quality of care, and friendliness. Recommend ideas within clinic leadership to improve patient experience and care. Communicate regularly within the team to support excellent patient care. Build and maintain confidence and credibility with all employees. Maintain a positive work atmosphere with clients, co-workers, and managers. Perform the physical, cognitive, and environmental functions of the position as required. Comply with Company standards of operations and maintain confidentiality. Consult with patients through multiple communication channels. Assist with urgent care activities as schedule allows. Perform other duties as assigned by management. Experience Requirements and Preferences Basic Requirements: Education: Master’s degree in Nursing (MSN) and/or Master’s degree in Physician Assistant Studies (MMS) Experience: Nurse Practitioner (NP) / Physician Assistant (PA) with 1 year of dedicated experience in direct primary care settings as a provider. Proficient in providing comprehensive primary care, including chronic care management, annual wellness visits for adults and pediatrics, and preventive health measures. Understanding of HEDIS quality metrics related to preventative screenings, vaccinations and chronic conditions. Required to be proficient in performing pediatric exams and see children aged 3 years and up. Current License or Certification: License and DEA must be active, in good standing, and verifiable with the proper regulatory agency; Primary Care Providers must be able to treat all ages and must meet any credentialing requirements needed; and DOT certification required to be obtained within 120 days of employment and maintained during employment.
