Job Responsibilities Careers at DPH: Work that makes a difference! Embracing Service, Inspiring Innovation, Promoting Teamwork, Pursuing Excellence, and Advancing Equity Under the supervision of the TB Unit Manager, provides / guides TB service delivery for a TB Team in an assigned area. Plans, implements and evaluates direct nursing care and coordination of services to individuals and families receiving TB services within an assigned area. This position requires role-based access to the agency's defined surveillance system, Client Automated Record and Encounter System (CARES), and the Electronic Health Record (EHR) for surveillance activities, case reporting, verification of diagnosis and treatment status, and documentation of patient care activities. Management / Supervision Directs and supervises activities of the TB Team to include: assigning referrals, assuring appropriate staffing/utilization of team members, coordinating with other TB Team Leads, serving as back-up to the Team, utilizing team building and conflict resolution, assuring orientation of new staff, assessing staff competencies, and providing input into staff evaluations. Case Management Provides holistic care by assessing patient/family resources, community resources, barriers to care, and utilization of services to determine an appropriate plan of care for the patient with Latent TB Infection (LTBI) or TB disease. Ongoing evaluation of care during treatment with the patient, TB Team, and the patient's primary care provider. Conducts contact investigations. Completes physical assessments and health histories of patients according to TB program guidelines. Obtains necessary lab work including, but not limited to, Complete Blood Count (CBC), liver function tests, Interferon‑Gamma Release Assays (IGRAs), Human Immunodeficiency Virus (HIV) tests, and sputum specimens. Provides treatment and/or administration of medication under a physician's order or standing order. Provides counseling and education regarding TB testing, evaluation, and treatment for Latent TB Infection (LTBI) or TB disease to patients and external partners. Administrative Duties Participates in Continuous Quality Improvement (CQI) activities such as record reviews, program evaluations, team meetings, etc. Maintains records and reports in accordance with agency standards including documentation in the patient record, the agency's time reporting systems, weekly reports, etc. Emergency Response/Special Projects Performs other related duties and special projects as assigned, including required travel if requested, and responds as directed during emergencies which may occur outside normal work hours, and travel if requested. Other Duties As Assigned Works as a member of a statewide interdisciplinary team. Maintains professionalism and willingness to assist where needed, even if the request is outside of the programmatic area. Minimum And Additional Requirements State Minimum Requirements Licensure as a registered nurse by the State Board of Nursing for South Carolina. Some positions require certification in Basic Life Support. Some positions may require possession of a valid driver’s license. Graduation from an accredited school of nursing. Agency Additional Requirements If compact state license, must obtain SCBON endorsement upon hire. Employee will obtain Basic Life Support (BLS) / Cardiopulmonary Resuscitation (CPR) / First Aid certification within thirty (30) days of hire or provide documentation of satisfactory course completion prior to employment. Must have a valid driver's license. Must be able to make home visits. Must have a valid driver's license; may be required to use personal vehicle. May need to use stairs to enter client homes; may encounter domestic animals. Significant travel and/or overnight stay within the state may be required. Must be able to bend, stoop and lift thirty (30) pounds. Must be able to pass a fit-test and be able to use appropriate respiratory protective equipment; employee will be in contact with infectious or potentially infectious tuberculosis (TB) cases/suspects. Must participate in disaster preparedness during hazardous weather or declaration of an emergency, subject to duty and/or call on a 24-hour basis. May require flexible work hours. Institutions of Higher Learning must be recognized by the Council for Higher Education Accreditation. Preferred Qualifications Bachelor of Science in Nursing (BSN) degree. Five (5) years of nursing experience. One (1) year of supervisory experience. Bi‑lingual (Spanish/English) helpful. Tuberculosis case management experience. Experience with electronic health record (EHR); technology databases, applications, and record-keeping.
