• Reason for Need: Locum coverage needed while team recruits a full-time permanent NeuroSurgery MD/DO.

• Candidate current CVComplete SMG Locum screening formCandidate MUST board certified.

• If DEA is not currently registered in VA, please list intention to acquire DEA such as changing address of current DEA or purchasing an additional DEA.

• Candidates must have completed YOUR locum credentialing/vetting process.
• ALL Malpractice claims must be disclosed.
