RN Admissions Nurse Hours 11am-7pm; 12pm-8pm Job Summary: We are seeking a compassionate and skilled RN Admissions Nurse to join our dedicated team of healthcare professionals at Absolut Care of Allegany in Allegany, NY . As a key member of our admissions team, you will play a vital role in ensuring a seamless transition for new residents, providing exceptional care, and promoting a warm, welcoming environment. About the Role: RN Admissions Nurse is responsible for conducting thorough assessments of new residents, gathering medical history, and developing personalized care plans in collaboration with our healthcare team. You will coordinate with our interdisciplinary team to ensure timely and effective care, communicate with residents' families, and provide education on our services and programs. Your attention to detail, strong clinical skills, and excellent communication abilities will enable you to navigate complex health challenges and build trust with our residents and their families. Responsibilities: Conduct initial assessments, evaluations, and documentation for new residents, including identifying needs and developing individualized care plans. Collaborate with the healthcare team, including physicians, therapists, and other caregivers to ensure comprehensive care. Communicate effectively with residents, families, and healthcare professionals to ensure a safe and supportive environment. Monitor and report changes in residents' conditions, documenting assessments and interventions as required. Participate in quality improvement initiatives to enhance the overall care experience.
