RN - Case Manager

Providence Saint Joseph Medical Center - Burbank is seeking an RN - Case Manager. This travel position requires a case manager with 2 years of acute medical care manager experience performing care coordination and discharge planning. The shift is from 08:00 AM to 04:00 PM, with 5 shifts per week for 40 scheduled hours. The contract duration is 13 weeks, starting ASAP. Weekend coverage is typically one full weekend per 4 weeks, subject to department need. The candidate must have a CA state license or a pending license that must be obtained by the start date. Travel is required, and the candidate must commit to working 3 of the 5 holidays (Thanksgiving, Xmas Eve, Xmas Day, NYE, NYD). Two references covering 1 year within the last 3 years are required. Local radius is 50 miles, and local rule is accepted at a lower BR. Float between all facilities within the state is expected. Communication devices will be provided or reimbursed up to $50.00 per month. Modules are non-billable and completed pre-start and annually.
