Key Responsibilities\n

  • Conduct comprehensive psychosocial assessments to evaluate patients' emotional, social, and financial needs, ensuring tailored treatment plans.

  • Provide both individual and group counseling sessions, addressing urgent issues related to adjustment, grief, coping strategies, and recovery pathways.

  • Collaborate with interdisciplinary medical teams, including physicians and nurses, to integrate psychosocial care within overall patient treatment plans.

  • Coordinate with community organizations to facilitate access to ongoing support services post-discharge, ensuring continuity of care and reducing the risk of relapse.

  • Deliver crisis intervention and emotional support to patients and their families, helping them navigate challenging situations with empathy and professional guidance.

  • Develop and implement detailed discharge plans, connecting patients with appropriate community resources and services tailored to their ongoing recovery needs.

Qualifications\n

  • Master’s degree in Social Work from an accredited institution required.

  • Active state licensure as a Licensed Clinical Social Worker (LCSW).

Compensation and Benefits\n
