Physical Therapist

We are seeking licensed Physical Therapists to provide personalized, one-on-one care to older adults in their homes and senior living communities. Key Responsibilities Include Conducting evaluations and developing individualized treatment plans Delivering evidence-based therapy to improve mobility, independence, and overall well-being Documenting patient progress accurately within electronic medical records Collaborating with interdisciplinary teams including PTs, OTs, SLPs, and fitness specialists Educating patients, caregivers, and staff on safety and aging in place strategies Providing telehealth and virtual care to enhance access and flexibility Qualifications And Skills Required Valid Physical Therapy license or eligibility in the practice state Degree from an accredited Physical Therapy program Proficiency in basic computer use and EMR systems Current CPR certification Physical Demands Include Performing moderate physical effort, including lifting up to 50 lbs occasionally Assisting with patient transfers and mobility Standing, walking, climbing stairs, and driving between patient locations Using PPE as required This role offers flexibility, competitive compensation, benefits, and opportunities for career growth within a supportive, clinician-driven environment. #J-18808-Ljbffr
