Travel RN Case Manager - $3,086 per week

Job Overview TNAA TotalMed RN is seeking a travel nurse RN Case Manager for a travel nursing assignment in Carmichael, California. Responsibilities As an RN Case Manager, you will work with a highly skilled team of professionals to advocate for patients, evaluate, plan, provide resources, and facilitate communication with family members. You will be responsible for comprehensive coordination of short‑ and long‑term care for patients, with the goal of decreasing hospital stay lengths and reducing readmission rates to generate cost‑effective outcomes. Evaluate and assess new patients. Create and update comprehensive short‑ and long‑term patient care plans. Educate families and patients about resources and health care options. Collaborate with other RNs, doctors and social workers. Properly document and submit medical records. Communicate with insurance companies to collaborate or appeal decisions. Improve financial status by analyzing results, monitoring variances, identifying trends and recommending actions to management. Requirements BLS certification. 2+ years of recent case management experience. Current RN license. Dedicated from 04/28/2024 for 13 weeks . Travel nursing employment. 40 hours per week, 8‑hour day shifts. * Additional certifications may be required before beginning an assignment. Physical Requirements Ability to stand and walk for long periods, carry up to 10 pounds, bend, lean and stoop without difficulty. Manage a fluctuating caseload during times of high census and work efficiently in a collaborative environment.
