Key Responsibilities

• Provide physical therapy evaluation and treatment services to patients in a skilled nursing facility setting
• Float to a sister location as needed to support patient care continuity
• Develop and implement individualized treatment plans based on patient needs and goals
• Document patient progress, treatment plans, and outcomes in accordance with facility and regulatory standards
• Collaborate with interdisciplinary care teams to coordinate patient care

Required Qualifications

• Active Physical Therapist (PT) license in the state of New Hampshire
• Graduate of an accredited Physical Therapy program
• Ability to float between multiple facility locations as required

Preferred Experience / Skills

• Prior experience working in a skilled nursing facility or long-term care setting
• Experience with geriatric patient populations
• Familiarity with electronic medical records (EMR) documentation systems
