Hospice Admissions & Care Coordinator

Compassus is seeking a Hospice Admissions Coordinator to process patient referrals, coordinate admissions, and enter admission data locally. The role operates Monday through Friday, 8am to 5pm, with an emphasis on communication with physicians and hospitals to secure orders for admission. Applicants should have a high school diploma or GED; a Bachelor's degree is preferred and LPN is advantageous.
