CP Rochester and Happiness House are private nonprofit health, educational, residential and human service organizations that have been serving communities in the Finger Lakes Region since 1969 and the Greater Rochester Area since 1946. Today with sites in Rochester, Canandaigua, Geneva, Gorham, Midlakes and Waterloo, our agencies offer an array of educational, therapeutic, recreational, residential and family support programs and services for children and adults with and without disabilities. On average, our agencies serve 3,650 families each year and employ approximately 600 people.

Our staff consists of dedicated and knowledgeable professionals and clinicians with expertise in the areas of Special Education, Early Childhood Education, Physical, Occupational, Speech and Music Therapies, Psychology, Service Coordination, Nursing, Dental Services, and Developmental Disabilities.

Description

Job Title: TBI Program Assistant Substitute

Location: Canandaigua site

Salary : $17.50 - $18.50 per hour

The listed base pay range is a good faith representation of current potential base pay for successful applicants. It may be modified in the future. Pay is determined by factors including experience, relevant qualifications, internal equity, and location.

Employment : Substitute/12 Month position working as needed. Program hours are Monday through Friday, 8:30 am - 3:30 pm

Summary/Objective : Ensure safety and supervision of all individuals focusing on fulfilling the Agency's Mission, Vision and Values and providing services which meet or exceed the goals of the individual, department and the agency

Minimum Qualifications

1. High School Diploma
2. One year of experience providing services to individuals with disabilities, preferably working with individuals with traumatic brain injury.
3. Knowledge of community resources and means of accessing these resources.
4. Valid NYS driver's license.
5. Demonstrates basic reasoning and writing skills.

Responsibilities include:

1. Imparts agency philosophy of "Equal Opportunity, Independence and Realization of Individual Potential" to participants, families, program staff and community resources.
2. Maintains confidentiality of information relating to individuals and their families in accordance with agency policy.
3. Participates in conferences, workshops, department meetings, in-service and on-line training as needed or recommended to enhance job skills and personal growth and development.
4. Adheres to agency's policies and procedures as well as department guidelines.
5. Accesses and maintains passwords and log ins for all required electronic health records, agency email, virtual trainings, and electronic employee records as required by the program.
6. Functions as the primary assistant to the Program Specialists.
7. Reviews each individual plan, safeguards/PPO and any other relevant information related to the care of individuals at least annually and as changes occur.
8. Implements each program participants' therapeutic and behavioral plans.
9. Works with individuals on special instructional projects to achieve goals on each individual's program plan.
10. Provides the Program Specialist/ Supervisor with information about program participants, which will assist in the development of appropriate program experience for all participants.
11. Completes required documentation about individual's performance according to established guidelines and timeframes.
12. Assists in the development of instructional materials.
13. Assists the Program Specialist in related instructional work as required.
14. Assists in maintaining the cleanliness and the orderly appearance of program equipment, supplies and program areas.
15. Attends all meetings as required and appropriate.
16. Leads planned small group activities including but not limited to in house activities and community activities.
17. Drives the agency vehicles as required to facilitate community-based activities.
18. Operates wheelchair lifts and ramps safely and in accordance with operating procedures in agency vehicles.
19. Maintains a safe environment at Happiness House and in the community.
20. Provide the participants with assistance on ADL's including but not limit4ed to, recognizing safety hazards, transferring, toileting, changing, ambulating and feeding as required by individual need.
21. Functions as a positive, contributing member of the TBI/NHTD/OPWDD program team.
22. Performs other duties at the discretion of the Program Supervisor and Director or his/her designee.
23. Supervise the program participants at the program site and community locations.

The employee frequently is required to sit, stand; walk; use hands to finger, handle or feel; and reach with hands and arms. The employee must frequently lift and/or move objects up to 10 pounds and occasionally lift and/or move objects up to 30 pounds.

Travel: Travel is primarily local during the business day.
