About the Role:

As a Social Services professional at the Skilled Nursing Facility you will play a vital role in making a meaningful difference in the lives of our patients and their families. Our team is committed to providing exceptional care and support, and we are seeking a dedicated and compassionate individual to join our mission.

Responsibilities:

Provide emotional support and counseling to patients and their families to address their social, emotional, and psychological needs
Develop and implement individualized care plans to promote patient well-being and independence
Facilitate patient and family communication with healthcare providers and other stakeholders
Conduct assessments and evaluations to identify patient needs and develop strategies for addressing those needs
Collaborate with other healthcare professionals to ensure comprehensive care and support
Document patient progress and maintain accurate records

Requirements:

Bachelor's degree in Social Work, Psychology, or a related field (or equivalent experience)
