Travel Nurse RN - Case Management - $2,674 per week

Care Career is seeking a travel nurse RN Case Management for a travel nursing job in Pittsfield, Massachusetts. Job Description & Requirements Specialty: Case Management Discipline: RN Start Date: 09/07/2026 Duration: 13 weeks 40 hours per week Shift: 8 hours, days Employment Type: Travel Position: RN Case Manager Shift: Days – 7:00 AM–3:30 PM Schedule: 5 × 8-hour shifts Guaranteed Hours: 40 hrs/week EMR: Meditech Expanse – Highly Preferred Rate: $90/hr Responsibilities Manage healthcare needs and coordinate quality and cost-effective outcomes for patients across the continuum of care. Perform admission, concurrent, and discharge assessments . Coordinate and monitor services and resources using a multidisciplinary team approach. Evaluate appropriate level of care and resource utilization throughout hospitalization. Develop and coordinate transition and discharge plans . Arrange appropriate post-hospital care and community resources. Collaborate with physicians, patients, families, insurance organizations, and government agencies. Apply knowledge of Medicare, Medicaid, commercial payers, and reimbursement . Requirements 3 years clinical experience in acute care – Required 3 years experience in Rehab, Home Health, or Long-Term Care may be considered Massachusetts RN License – Required BLS – Required RN Case Management experience – Required Ability to perform comprehensive admission and discharge risk assessments. CCM/ACM – Preferred Meditech Expanse – Highly Preferred Strong communication, collaboration, negotiation, organizational, and analytical skills. Must work five 8-hour shifts ; 10/12-hour shifts are not available. Must reside more than 50 miles from the facility . Quick Offer.
