Location: 414 South Jefferson Street Napa, California 94559 Contract Length: 13 weeks + possible extension
Shift: 5x8hr Days (40 hours)

Job Description:
Outpatient RN Case Manager - Napa, CA Licensed Adult Day Health program serving frail elderly, younger disabled and people with Alzheimer's Disease and related dementia. Licensed for 100 a day with current average daily census of 72. RN role provides chronic disease case management, monitor medical conditions in conjunction with IDT and PCP in the community, and provides caregiver education and support. Goal of the program is to reduce ED and hospitalizations as well as prevent premature nursing home placement. EMR is Turbo TAR which is specific to CA ADHC industry so not likely anyone will know this EMR.
Dates: ASAP - Shift: 5x8hr Days - Years of Experience REQ: 1 year of prior RN experience REQUIRED (Clinic, Primary Care, Pre/Post, Home Health settings(s) preferred) - First-Timers Accepted: Yes - Weekend REQ: No - Certs REQ: BLS - Is on-call REQ? No - Will clinician float within scope to meet facility needs? No - Open to accommodating block schedule? No - Locals Accepted: Yes; - Pending License Accepted: Yes (CA license REQUIRED at time of start) - RTO Restrictions: Confirm with XXXX - Guaranteed Hours: Facility may call off traveler UP TO 1 shift per pay period (every 2 weeks) SPECIAL REQUIREMENTS - Candidate is expected to have reliable transportation and no additional scheduling restrictions. All travel pairs will be required to have their own transportation.
