Position Summary
The C.W. Williams Community Health Center is seeking an experienced Registered Nurse (RN) to provide high-quality nursing care in a primary care and community health setting.
The Registered Nurse works with physicians, advanced practice providers, nurses, medical assistants, patients, families, and community partners to support patient care and health services.
The ideal candidate has experience in primary care, public health, community health, or a similar outpatient healthcare setting and is committed to providing high-quality care to patients with a range of medical needs.
Experience in infectious disease prevention and treatment is strongly preferred , including experience with HIV/AIDS, sexually transmitted infections (STIs), communicable diseases, or other infectious disease programs.
The RN also supports quality improvement, patient safety, care coordination, health education, and the use of clinical data to improve patient care and meet health needs in the community.
Key Responsibilities
Patient Care & Clinical Services
- Provide safe, timely, and compassionate nursing care.
- Assess patients and identify changes in health status.
- Report significant patient findings to physicians and other members of the care team.
- Carry out patient care plans in collaboration with providers and other healthcare professionals.
- Provide education about medications, treatment plans, preventive care, disease management, and follow-up care.
- Monitor patient response to treatment and report changes in condition.
- Assist providers with examinations, procedures, treatments, and other clinical services.
- Provide telephone and in-person clinical triage within the scope of nursing practice and established protocols.
- Support preventive care and chronic disease services.
- Help patients follow treatment plans and obtain needed follow-up care.
- Respond to patient and family questions and concerns in a timely and professional manner.
Public Health & Infectious Disease
- Provide nursing services that support public health and community health programs.
- Support screening, prevention, treatment, education, and follow-up for infectious diseases.
- Follow infection prevention and control practices.
- Provide patient education about communicable disease prevention, treatment, medication use, and risk reduction.
- Support HIV/AIDS, STI, and other infectious disease services as assigned.
- Work with providers, public health programs, community organizations, and other service providers to support patient care.
- Help patients connect with medical, behavioral health, and social services when needed.
Documentation & Care Coordination
- Maintain accurate, timely, and complete documentation in the electronic health record (EHR).
- Review patient records and share relevant clinical information with members of the care team.
- Coordinate referrals, follow-up appointments, laboratory testing, medications, and other services as appropriate.
- Help patients understand their care plans and next steps.
- Help patients address challenges that may make it difficult to obtain or continue care.
- Protect patient privacy and follow HIPAA and other applicable requirements.
- Communicate clearly with patients, families, providers, and other healthcare professionals.
Quality & Patient Safety
- Participate in quality improvement and patient safety activities.
- Use clinical information and data to identify ways to improve patient care and outcomes.
- Follow clinical guidelines, organizational policies, and applicable regulatory requirements.
- Assist with activities related to Uniform Data System (UDS) reporting and quality measures.
- Support activities related to patient care quality, access to care, and health outcomes.
- Participate in Patient-Centered Medical Home (PCMH) activities and other quality programs as assigned.
- Identify and communicate ways to improve clinical processes and patient services.
- Report patient safety concerns and other clinical issues promptly.
Teamwork & Communication
- Work closely with physicians, advanced practice providers, nurses, medical assistants, behavioral health staff, and other members of the care team.
- Provide clinical support to coworkers within the scope of nursing practice.
- Communicate clearly with staff across departments.
- Participate in staff meetings, clinical meetings, training, and continuing education.
- Build professional working relationships with patients, families, coworkers, and community partners.
- Perform other duties as assigned.
Required Qualifications
- Associate Degree in Nursing (ADN) or Bachelor of Science in Nursing (BSN).
- Current, unrestricted Registered Nurse (RN) license in North Carolina.
- Minimum of 2–3 years of clinical nursing experience.
- Experience in primary care, public health, community health, or a similar outpatient healthcare setting required.
- Current Basic Life Support (BLS) certification required.
- Experience using electronic health records (EHRs).
- Strong patient assessment and clinical judgment skills.
- Ability to recognize changes in patient condition and take appropriate action.
- Ability to communicate clearly with patients, families, providers, and other healthcare staff.
- Ability to work independently and as part of a healthcare team.
Preferred Qualifications
- Experience in infectious disease prevention and treatment strongly preferred.
- Experience with HIV/AIDS, sexually transmitted infections (STIs), communicable diseases, or other infectious disease programs preferred.
- Experience working in an FQHC or community health center preferred.
- Experience with public health programs or care coordination preferred.
- Experience serving patients with complex medical and social needs preferred.
- Experience with UDS reporting, quality improvement, PCMH standards, or healthcare quality measures preferred.
- Bachelor of Science in Nursing (BSN) preferred.
- ACLS certification preferred.
- Experience with Athenahealth and/or CareWare preferred.

Work Environment
The Registered Nurse works primarily in a clinical healthcare setting and has regular contact with patients, families, providers, and other healthcare staff.
The position may involve:
- A busy work setting with frequent interruptions and changing priorities.
- Potential exposure to bloodborne pathogens, communicable diseases, bodily fluids, and hazardous materials.
- Use of appropriate personal protective equipment (PPE).
- Interaction with patients who may have complex medical or behavioral health needs.
- Moderate to high noise levels related to clinical equipment, patient activity, and staff communication.
Work schedules may occasionally include evenings, weekends, holidays, or other schedule changes based on patient and organizational needs.
