Minimum Requirements

• Bachelor's, Master's or Doctoral Degree in Physical Therapy from accredited schools approved by the APTA
• Active State License is Required to Start the Assignment
• BLS Certification May Be Required from AHA or ARC
• Illinois license required
• EMR: Cedaron
• Must have a verifiable permanent address 50 miles or more from facility

• Schedule options include 4x10s or 5x8s
• No weekends
