Allied Technician - Physical Therapist

Physical Therapist (PT)

We are seeking a dedicated Physical Therapist to join our team. The ideal candidate will have experience in both Skilled Nursing Facilities (SNF) and Home Health settings, with a commitment to providing high-quality patient care.

Key Responsibilities

• Provide assessment and therapeutic interventions in SNF and Home Health settings.
• Supervise and assist Physical Therapy Assistants (PTAs) to ensure effective treatment plans are followed.
• Manage a varied caseload, approximately 50/50 between SNF and Home Health, with flexibility to adjust as needed.
• Occasionally provide coverage in additional locations for SNF or Home Health services as required.
• Maintain productivity expectations, seeing an average of 6 patients per day for Home Health, equating to approx. 28 patients per week at optimal productivity levels.
• Utilize electronic record systems, including Net Health for SNF and HCHB for Home Health, to document patient progress and treatment plans.
• Collaborate closely with the therapy team and communicate effectively with patients and their families regarding care plans and progress.

Qualifications

• Bachelor's, Master's, or Doctoral degree in Physical Therapy from an accredited program.
• Professional licensure as a Physical Therapist in the relevant state.
• Current Basic Life Support (BLS) certification.
• Minimum of 1 year of experience in a clinical setting preferred; new graduates are welcome to apply.
• Strong communication and interpersonal skills for effective collaboration and patient engagement.
• Ability to work independently while following established guidelines and protocols.

Work Schedule

This position requires a commitment of approximately 36 hours per week, typically spread across five days with eight-hour shifts. Flexibility to accommodate patient needs and occasional location coverage is essential.

Additional Information

• Onsite support from a Scheduler and Contact person; therapy supervisor is currently on maternity leave.
• No on-call or callback requirements.
