Home Health Registered Nurse (RN)

We are seeking a dedicated and experienced Home Health Registered Nurse (RN) to join our team. The ideal candidate must possess home care charting experience along with a background in home health care.

Key Responsibilities

Provide nursing care and support to patients in their homes.
Conduct start of care assessments, recertifications, and daily visits.
Manage a caseload of approximately 30 adult patients, ensuring quality care and adherence to care plans.
Collaborate with clinical manager and educators for ongoing support and emergency assistance.

Qualifications

Current RN license in the state of work.
A minimum of 1 year of experience in a MedSurg floor in a hospital setting.
Experience with home care charting, specifically Homecare Homebase (HCHB).
BLS certification required.

Work Schedule

Typical shifts include 5-8 hour shifts, 4-10 hour shifts, or 3-12 hour shifts.
Weekend rotation is dependent on the nursing staff availability.
Holiday rotation participation is expected.

Additional Information

Clinical support is available 24/7 for emergencies and 8-5 PM for other inquiries.
Scrubs are required; no specific color requirement.
Orientation is provided with 2 weeks of preceptorship, including visits with a work coach.
Required equipment, such as a phone and tablet, will be provided.

This position focuses solely on home health and does not involve hospice care.

Coverage Area

Candidates must be available to cover the following counties: Marion, Washington, Taylor, LaRue, Hardin, Meade, and Breckenridge.
