RN Home Health
Provide clinical services to patients from birth to over 100 years in their homes. Assess patient conditions, implement care plans, and provide ongoing education and support to patients and their families. Perform admissions process and case management responsibilities in accordance with policies and standards of practice. Utilize thorough and timely electronic documentation and interdisciplinary communications.

Must-Have: 1 year Certified Home Care experience 1 year Case Management Experience Reliable, independent means of transportation to make home visits Willingness to work weekends, holidays, and on-call in accordance with department need/rotation Weekend rotations occur once every 6 weeks
