Case Manager
We are seeking a dedicated Case Manager to join our team. The ideal candidate will have a minimum of 4 years of experience in case management. Responsibilities include coordinating patient care, facilitating communication among healthcare providers, and ensuring patient needs are met effectively. The Case Manager will work collaboratively with interdisciplinary teams to develop and implement care plans. A focus on quality care and patient advocacy is essential in this role. Candidates must possess an active RN state license and relevant certifications. Strong organizational and communication skills are required.
