We are seeking dedicated Licensed Professional Counselors (LPC) for full-time and part-time roles within our innovative outpatient clinic. • Collaborate with primary care providers to identify and address mental health and psychosocial issues, contributing as an integral part of the healthcare team.
• Conduct comprehensive patient assessments focusing on diagnostic and functional evaluations, and provide treatment recommendations to physicians.
• Implement targeted interventions for patients in need of mental health treatment, tailoring approaches to individual client needs.
• Guide primary care providers in determining appropriateness of referrals for specialized mental health services, ensuring that patients receive optimal care.
• Maintain a system for follow-up procedures to track patient progress, adjusting treatment plans as necessary to facilitate recovery.
• Develop relapse prevention strategies to support patients in maintaining emotional stability and address potential crises before they escalate.
• Identify at-risk patients and collaborate in creating preventive care plans to manage their conditions effectively.
• Oversee and coordinate behavioral healthcare services, ensuring smooth communication and collaboration with both internal and external resources.
• Document patient progress and diagnostic information within treatment charts accurately, adhering to all relevant protocols.
• Keep primary care providers informed of patient status, fostering an environment of open communication and teamwork in treatment planning.
• Facilitate behavioral change in patients facing physical and mental health challenges, helping them to adopt healthier lifestyle choices.
• Equip patients with self-management techniques and educational resources, empowering them to take an active role in their treatment and recovery journey.
• Provide consultation on behavioral health services to clinic management and fellow staff, offering insights for service improvements.
• Assist patients in adhering to medical treatments prescribed by their primary care providers, including managing side effects and understanding medication regimens.
• Uphold the highest ethical standards and follow the Code of Ethical Conduct for Clinical Professionals in every aspect of your work.
• Participate in any other duties as assigned to support clinic objectives.

This role primarily focuses on veteran clients, as well as their families, who may be experiencing a variety of mental health challenges, including PTSD, anxiety disorders, and depression. • A minimum of a Master’s Degree in Clinical Counseling or a related field is required.
• Possession of a current Licensed Professional Counselor (LPC) license is mandatory.

• Demonstrated knowledge of DSM-IV diagnoses and developmental psychopathology is essential.
• Physical and mental resilience to meet the demands of a fast-paced counseling environment.
• Comprehensive awareness of how various physical and mental health issues affect individuals, families, and the wider community.

• Bi-lingual proficiency in English and Spanish is preferred, aiding in better communication with diverse clients.
• Familiarity with community resources and the ability to navigate these services for client referrals.
• Experience in outpatient behavioral health settings will be highly regarded.
