Description

As a Case Manager, you will coordinate patient care and ensure that patients receive appropriate medical services. You will work with healthcare providers to assess patient needs, develop care plans, and facilitate access to resources. This role is crucial in improving patient outcomes and ensuring compliance with healthcare regulations. Requirements

• RN License required, nursys must be on profile
• BLS required
• HWL clinical prescreen with Manager having final interview and offer
• ALL RTO must be on submission
• Radius Rule 50 miles from perm address
• Hard document deadline is Monday prior to start
• Degree accreditation or 5 years of experience required
• FLOATING REQUIRED, Travelers are first to float
• 2 years of Case Manager experience required
