Join to apply for the Admissions Nurse (RN) role at EverHeart Hospice 3 weeks ago Be among the first 25 applicants Location:Western & Central Ohio Schedule:9:00 AM – 9:30 PM(set template days) Weekend Rotation:Every 4th weekend(Friday & Monday off after worked weekend) Holiday Rotation:2 holidays per year Hours:Full-Time(36 hours/week) Pay:$35.00 – $40.00/hour(based on education, experience, and certifications) Position Summary EverHeart Hospice is seeking a compassionate Admissions Registered Nurse (RN) to manage hospice referrals, intake, and patient admissions across Western and Central Ohio. This role conducts comprehensive admission assessments, determines hospice eligibility, and educates patients and families on hospice and palliative care services. Qualifications Minimum 2 years RN experience (Med/Surg, Oncology, or Community Health preferred; hospice a plus) Valid driver’s license and reliable transportation Strong communication and time management skills Ability to work independently Admissions RN Responsibilities Conduct hospice admission assessments and verify eligibility Manage referrals, intake, and admissions Educate patients and families on hospice, palliative care, and insurance benefits Identify appropriate palliative interventions and ABN use Process physician and Medical Director orders Coordinate Durable Medical Equipment (DME) Provide skilled hospice nursing care, symptom management, and emotional support Support clinical operations during low admissions (home visits, end-of-life care, chart reviews) Ensure compliant, accurate documentation Medical, dental, and vision eligibility 1st of the month following hire 21 paid days off per year (PTO, sick time, and holidays) 401(k) eligibility 1st of the month following hire Employer-paid short-term disability Mileage & cell phone reimbursement Monthly employee recognition events PayActiv—early access to earned wages $200 initial clothing allowance for full-time positions Seniority level Entry level Employment type Full-time Job function Health Care Provider Industries Hospitals and Health Care Referrals increase your chances of interviewing at EverHeart Hospice by 2x #J-18808-Ljbffr
