Position Summary We are seeking an experienced and empathetic Arizona Nurse Practitioner (NP) to join our team as a Remote Care Manager. In this role, you will engage with patients virtually to assess, coordinate, and monitor their health care needs—playing a vital part in our mission to enhance quality of life through proactive care management. Key Responsibilities Conduct remote patient assessments with empathy and professionalism Monitor patients' health status and elevate warning signs appropriately Apply medical terminology knowledge to assess and coordinate care Communicate clearly and compassionately with patients and families Document thoroughly and accurately within multiple computer systems Prioritize daily responsibilities while working independently Provide excellence in patient care and customer service Maintain patient confidentiality and HIPAA compliance at all times Qualifications Current, active Arizona NP license in good standing Minimum 3 years of experience in a clinical or patient-facing setting Minimum 3 years of experience in care management or case coordination Prior experience working remotely preferred Superior verbal communication and active listening skills High proficiency in medical terminology Excellent organizational and documentation skills Ability to recognize clinical red flags and elevate care as needed Tech-savvy; able to work within multiple systems efficiently Must have a HIPAA‑compliant, private work area at home Flexible, remote work environment Opportunities for professional growth The chance to make a real impact on patients’ lives Job Type Part‑time 401(k) Health insurance Health savings account Life insurance Paid time off Application Question(s) Do you have a minimum 3 years of experience in a clinical setting or patient‑facing environment?
