Job Summary:
Outreach Care Coordinator provides client-centered, trauma informed street outreach support and services to people experiencing homelessness to assist the individual in achieving their goals. This includes housing assistance, providing basic needs/supplies, direct referrals for physical health, mental health, and substance abuse services, transportation assistance, benefit assistance, and providing information on where to access services throughout Clark County.
The primary focus of this position is to provide injury prevention and drug checking services through low-barrier, participant-centered engagement. Injury prevention services include the distribution of safer use supplies, naloxone, education on overdose prevention, infectious disease prevention, and referrals to health and social services. Drug checking services emphasize residue testing, providing participants with timely and accurate information regarding test results, and education to support informed decision-making.
Outreach Care Coordinator will provide resources to improve the health and housing outcomes of unhoused persons. While injury prevention and drug checking are the primary responsibilities of this position, the coordinator will also provide outreach, housing navigation, and care coordination services to support participants and program operations as needed. This position will allow for flexible scheduling to meet the needs of clients and the facility and could include evening and weekend hours.
Knowledge, Skills and Abilities:
- Knowledge of challenges faced by those living in homelessness
- Skilled in crisis intervention techniques
- Ability to build positive and trusting relationships with diverse populations, particularly people experiencing serious and persistent mental illness and substance use disorder
- Ability to participate in program evaluation
- Ability to establish and maintain professional working relationships with community members, business owners, funders, government officials, and law enforcement
- Knowledge of evidence-based injury prevention practices
- Knowledge of infectious disease prevention strategies related to HIV, viral hepatitis, and sexually transmitted infections
- Knowledge of residue drug checking procedures and participant education
- Ability to provide non-judgmental, culturally responsive services to people who use drugs
- Ability to safely handle biohazardous materials and follow Universal Precautions
- Ability to collect, document, and report program data required by multiple funding sources
Summary of essential job functions and responsibilities:
- Provide comprehensive injury prevention services through mobile outreach and community-based service delivery.
- Distribute injury prevention supplies, naloxone, and other wound care related materials in accordance with the Department of Health requirements.
- Conduct residue drug checking services including sample collection, residue testing, communicating results, and providing education regarding overdose risk reduction.
- Provide participants with accurate, non-judgmental information regarding residue testing results and available health resources.
- Maintain drug checking supplies, testing equipment, inventory, and quality assurance procedures.
- Maintain documentation and reporting requirements related to injury prevention, drug checking, participant encounters, referrals, and grant performance measures.
- Identify and implement innovative engagement strategies to reach currently unsheltered houseless community members
- Provide trauma informed outreach and engagement services to people experiencing homelessness
- Assist participants in linkage to desired supportive services and housing barrier reduction
- Maintain records and enter client engagement information into the HMIS database system
- Enter and maintain required injury prevention program data in Department of Health reporting systems and other applicable databases.
- Gather data and participant feedback to demonstrate outcomes of program
- Respond to crisis situations as needed
- Engage community members and local businesses to problem solve ways to effectively engage with people experiencing homelessness
- Attend staff meetings, workshops, and trainings
- Attend required Department of Health trainings, monthly Community Drug Checking Network (CDCN) meetings, and other grant-required meetings.
- Network in the social service community to better assist participants
- Assist with development and implementation of injury prevention and drug checking policies, procedures, workflows, and quality improvement initiatives.
- Provide operational support and program coverage during staff absences to ensure continuity of injury prevention and drug checking services.
- Perform related duties as assigned
Minimum Qualifications
- Two (2) years' experience working with people experiencing homelessness, substance use disorder, and various health challenges
- Knowledge of injury prevention practices and have the ability to provide high-quality non-judgmental services
- Experience working with people who use drugs and providing injury prevention or public health services preferred.
- Experience providing residue drug checking or willingness to complete required training.
- Knowledge of infectious disease prevention and overdose prevention practices preferred.
- Experience and ability to work with diverse communities
- Possess or be able to obtain CPR, blood borne pathogens, basic first aid certification, naloxone administration training
- Ability to utilize Microsoft Office Suite software
- Have or be able to easily obtain the skills to work with the HMIS database
- Ability to accurately collect, maintain, and report program data for multiple contracts and funding sources.
- Exhibit the ability to work as a collaborative and productive part of a team
- Must have valid driver's license
Work Environment and Physical Demands:
The nature of work involves working with people living in homelessness, including streets, woods, and under bridges. Staff should be able to lift a minimum of 25 lbs., be able to safely navigate steep terrain and drive large vehicles.
Occasional first aid may involve exposure to blood and other bodily fluids, and staff should be prepared to respond to opioid overdoses and administer naloxone.
Staff will routinely handle injury prevention supplies and may encounter used syringes and other biohazardous materials while providing mobile services. Employees are expected to follow Universal Precautions, agency safety protocols, and safe sharps handling procedures at all times.
Work may include operating mobile injury prevention services, transporting supplies, setting up community service locations, and supporting outreach activities in a variety of weather conditions.
