Registered Nurse (RN) Health Management Resource

Position Overview Department: 38913 Advocate Sherman Hospital – Health Management Resources (HMR) Status: Part time Benefits Eligible: No Hours per Week: 0 Schedule: Monday through Friday, 8:00am – 5:00pm Pay Range: $38.20 – $57.30 per hour Location: Sherman Hospital, Elgin, IL Job Description Registered Nurse (RN) Health Management Resource Per Diem. Work on an “as needed” basis during the 8:00am – 5:00pm shift. The primary focus areas include weight management, cardiac disease, and diabetes. The RN provides high‑quality, patient‑centered care through evidence‑based practice, clinical expertise, and compassionate delivery in an acute care setting. Responsibilities Engage in unit councils, professional governance, and quality initiatives to improve care processes and apply evidence‑based practices. Utilize the nursing process (assessment, planning, diagnosis, implementation, evaluation) and involve patients and families throughout the continuum of care. Monitor patient conditions, adjust care plans, mobilize resources, and collaborate with the care team to influence outcomes. Uphold and promote a culture of safety and continuously evaluate patient, team, and unit outcomes, taking action as needed. Administer medications, treatments, and therapies safely and in accordance with clinical protocols and procedures. Demonstrate effective communication, provide feedback, resolve conflicts, and foster team collaboration with appropriate delegation. Pursue professional development, complete required education, and maintain certifications. Adhere to the ANA Code of Ethics, practice ethical decision‑making, respect interdisciplinary roles, and contribute to integrated, unbiased patient care. Delegate appropriately to RNs, LPNs, and unlicensed assistive personnel and maintain accurate, timely EHR documentation. Float to other units, departments, care areas, or facilities within the designated service area as needed to meet patient care demands. Demonstrate knowledge and skills necessary to provide age‑appropriate care, applying principles of growth and development across the lifespan. Qualifications Graduate of a Board of Nursing approved nursing education program. Active registered nurse (RN) license (multi‑state compact or single‑state) with privileges to practice in the state(s) where services are provided. Basic Life Support (BLS) certification. Minimum of 1 year of clinical nursing experience. Strong clinical judgment and critical thinking. Effective time management, prioritization, and problem‑solving skills. Excellent communication and interpersonal skills. Ability to work in a fast‑paced, dynamic environment. Proficiency in computer functions such as email, electronic records, and digital platforms. Physical Requirements and Working Conditions Must be able to sit, stand, walk, lift, squat, bend, reach above shoulders, and twist frequently during the workday. Must lift and support up to 35 pounds when handling patients, equipment, and supplies. Must be able to push/pull with 30lbs of force and perform a sliding transfer of 150lbs with a second person present. Must use fine hand manipulation skills for IV insertion and other procedures. Performs physical safety interventions such as restraint and verbal de‑escalation when required. May be exposed to chemical and hazardous waste, bloods and body fluids, and communicable disease; protective clothing and equipment must be worn as necessary. Must respond quickly to changes in patient and/or unit conditions; physical abilities testing may be required. Preferred Qualifications Bachelor of Science in Nursing (BSN).
