Position Summary

The Case Manager identifies the resources needed to address each program participant's individualized needs. This position takes the necessary actions to support implementation of the participant's service treatment plan, including advocating for access to needed services, identifying program needs and service gaps, and recommending appropriate strategies.

The Case Manager works from a holistic, team-based, and values-driven perspective while demonstrating a commitment to RHD's values in the performance of all job responsibilities.

What You'll Do

Team-Based Planning

• Participate as a member of a multidisciplinary team providing holistic, values-based services.
• Assist with the development and review of individualized recovery plans in collaboration with the program participant, clinical and medical staff, and other stakeholders identified with the participant.
• Help determine the resources and services needed to support each participant's individualized goals.
• Advocate for program participants to help ensure access to appropriate services and supports.
• Identify program needs and service gaps and recommend strategies for addressing them.
• Develop and maintain productive relationships with community agencies.
• Connect program participants with available community resources and services.

Direct Participant Support

• Provide community-based services to program participants.
• Educate participants about available services, supports, and resources.
• Assist participants with making meaningful connections in their communities.
• Support participants in working toward the goals identified in their recovery plans.
• Transport program participants to appointments as required.

Documentation and Administrative Responsibilities

• Complete assessments, service documentation, and other records accurately and within required timeframes.
• Maintain documentation in accordance with RHD requirements and applicable funder, licensing, and accreditation standards.
• Participate in program planning, coordination, and service-review activities.
• Perform other job-related duties as assigned.

Qualifications

Required

• Bachelor's degree in human services or a related field.
• At least two years of general work experience providing support to individuals receiving behavioral health, addiction recovery, or behavioral health home services.
• Intermediate computer skills.
• Valid driver's license.
• Ability to travel throughout the program's service-delivery area.
• Ability to transport program participants to appointments as required.

Preferred

• At least one year of case management experience.

Working Conditions

• This position provides services in community-based settings throughout the program's service-delivery area.
• Regular local travel is required.
• The Case Manager is required to drive program participants to appointments and other approved service-related destinations.

Physical Requirements

This position is classified as medium work and may require:

• Exerting up to 50 pounds of force occasionally.
• Exerting up to 20 pounds of force frequently.
• Exerting up to 10 pounds of force constantly to move objects.
• Standing or sitting for periods of time.
• Walking.
• Using hands and fingers to handle, operate, activate, prepare, inspect, or position objects.

• Performing repetitive motions.
• Reaching at high or low levels.

• Performing the essential functions of the position with or without reasonable accommodation.

Schedule

• Full-Time
• Monday-Friday 10am-6pm or 11am-7pm
• OR Tuesday-Saturday 9am-5pm or 8am-4pm
