This role focuses on creating a caring environment centered on healing and wellness. Nursing practices are based on evidence and current research, as well as hospital policies and procedures. Clinical decision-making encompasses all four phases of the nursing process, critical thinking, and creativity. Nursing excellence is promoted through collaborative relationships among nursing staff and other disciplines. Responsibilities include coordinating, ensuring, and providing direct patient care activities. This involves conducting nursing rounds, assessments, and treatments as applicable. Facilitating admissions, transfers, and discharges is also a key responsibility. Provision of patient and family-centered education is essential. Care coordination and individualized treatment planning are part of the role. Job Responsibilities: Assessment of Patients' Needs: Performs and documents comprehensive assessments in accordance with the scope of practice. Considers the patient’s culture, age, and developmental level. Identifies patient risk factors and presenting complaints. Obtains health information from families or others as needed. Collects objective and subjective data for assessment. Verifies initial admission assessments if conducted by other team members. Develops a problem list, prioritizing needs based on input from the inter-professional team. Designs actions and interventions relevant to the patient’s culture and age. Determines desired outcomes in coordination with the patient and family. Fully implements patient care therapies and procedures within the established guidelines. Explains procedures, treatments, and processes to patients and families. Initiates actions to alleviate patient stress and discomfort. Recognizes and communicates changes in patient conditions appropriately. Conducts ongoing assessments and documents responses to therapies according to policy. Delegates tasks appropriately and oversees team members in their performance of activities. Monitors accuracy and completeness of delegated patient care. Addresses educational needs of patients according to their level of comprehension. Facilitates the discharge process by executing orders in a timely manner. Provides input into patient assignments considering staffing and patient acuity. Demonstrates participation in unit-based activities supporting organizational values. Promotes a positive and healing environment through actions and words. Assures nursing excellence in accordance with accepted standards and evidence-based practice. Actively participates in department quality improvement activities and core measures. Uses a holistic perspective in regard to patient care.
