Family Home Health Network , is a provider of intermittent Home Health care, palliative care, hospice care and custodial private care services throughout the Chicagoland area.

The Occupational Therapist is responsible for providing occupational therapy services to patients including but will not be limited to; assessing and evaluating therapeutic/rehabilitative/functional status and participating in the development of the total plan of care. The Occupational Therapist will also instruct patients, families and caregivers in the use and care of therapeutic appliances and determine priority needs for occuptational therapy.
Qualifications:
Baccalaureate OR master’s program (preferred) in Occupational therapy approved by an accredited organization.
- Current Illinois license as an Occupational Therapist
- Current CPR card
- Valid IL driver’s license and auto insurance policy
- Home Care Home Base experience preferred
