Overview Generations Hospice is seeking a passionate, dedicated Hospice RN to join our team. This role offers lower patient-to-nurse ratios for meaningful, unrushed hospice care, strong interdisciplinary team support with social work, chaplaincy, aides, and providers, and dedicated after‑hours triage support. Homecare Homebase EMR streamlines documentation. Office Location: 1028 S. Main Street Suite C Fond du Lac, WI 54935. Coverage area: South Fond du Lac areas – Columbus, Fall River, Beaverdam, Mayville. Schedule: Monday–Friday 8:00 AM–5:00 PM. Responsibilities Assess and monitor physical, emotional, and psychological needs of patients. Create hospice care plans that align with patient wishes and goals. Administer medications, treatments, and interventions; provide pain and symptom management. Provide nursing care, pain and symptom management, and education/support to patients’ families and caregivers. Collaborate with interdisciplinary team. Maintain accurate and timely documentation.
