- Assess patients in both inpatient and outpatient settings and develop customized treatment strategies
- Implement physical therapy interventions focused on enhancing mobility, strength, balance, and overall function
- Work in collaboration with nurses, physicians, case managers, and various rehab professionals to promote optimal outcomes
- Provide education to patients and families concerning therapeutic goals, discharge planning, and home exercises
- Keep detailed records of evaluations, treatments, and progress in accordance with facility and regulatory standards
- Engage in care team rounds and interdisciplinary meetings to ensure a seamless care experience
- Doctorate in Physical Therapy (DPT) or a comparable degree from an accredited institution
- State licensure as a Physical Therapist
- CPR certification is preferred
- Specialized certifications (such as neurological or geriatric) are advantageous but not mandatory
- Previous experience is highly desirable
- Ability to treat patients with intricate medical and mobility challenges
- Excellent communication and teamwork abilities
- Familiarity with digital charting and clinical documentation systems
