Position Title: Physical Therapist – Outpatient
Location: Cortlandt Manor, NY 10567
Duration: 27-Week Contract Assignment
Job Type: Contract | Local or Travel
Schedule: 5x8.5-Hour Day Shifts (42.5 Hours/Week)

Shift Details:

• Monday: 12:30 PM – 8:30 PM

• Tuesday – Thursday: 12:00 PM – 8:30 PM

• Friday: 9:00 AM – 3:00 PM

• No Weekends

• 30-minute break included

Compensation:

• Local Pay: $54 – $60 per hour

• Travel Pay: $54 – $77 per hour

Job Summary

A leading healthcare facility in Cortlandt Manor, NY is seeking a licensed Physical Therapist (PT) for a 27-week outpatient assignment covering a full-time leave of absence. This role requires a strong outpatient background and the ability to work independently in a fast-paced clinical setting.

The Physical Therapist will evaluate, plan, direct, and administer therapeutic treatments to restore function and prevent disability following injury, illness, or surgery.

Key Responsibilities

• Perform comprehensive physical therapy evaluations

• Develop individualized treatment plans based on diagnosis and functional goals

• Administer therapeutic exercises, manual therapy, and modalities

• Monitor patient progress and adjust treatment plans as needed

• Complete timely documentation including evaluations, daily notes, progress reports, and discharge summaries

• Educate patients and families on treatment plans, home exercise programs, and injury prevention

• Collaborate with physicians and interdisciplinary teams to ensure coordinated care

• Maintain compliance with HIPAA, infection control, and facility policies

• Meet productivity standards and support quality improvement initiatives

• Utilize EPIC EMR for documentation

Required Qualifications

• Active New York State Physical Therapist License (Required)

• Strong knowledge of outpatient rehabilitation practices

• Ability to work independently in a fast-paced outpatient environment

Preferred Qualifications

• 1–2+ years of outpatient physical therapy experience

• Experience treating orthopedic, post-surgical, and sports-related injuries

• Prior hospital-based outpatient experience

• Experience with EPIC EMR (Highly Preferred)

• Strong manual therapy and patient education skills
