The Iowa Cross ACT program of Resources for Human Development (RHD) is an Assertive Community Treatment program based in Knoxville, Iowa , serving the seven-county CROSS mental health and disability services region.

Our multidisciplinary ACT team works together to provide coordinated, community-based services designed to support individuals with behavioral health needs in achieving greater stability, independence, and success.

What You'll Do

As an LPN , your primary role will be to provide oversight, coordination, and direct intervention related to the health needs of individuals participating in the program.

You will:

• Maintain accurate nursing documentation and medication records.
• Assist participants in learning about their medications.
• Ensure medications are taken as ordered by physicians and properly recorded in the electronic Medication Administration Record (eMAR).
• Provide nursing care and health education, including services such as diabetic counseling.
• Assess participants' health and emotional needs.
• Communicate and collaborate with nursing and program staff to coordinate participant care.
• Communicate serious health or program concerns to appropriate nursing and program leadership.
• Maintain participant confidentiality and protect private health information in all settings.
• Provide direct nursing support based on the individual needs of program participants.
• Perform additional duties as assigned.

Qualifications

Required

• Current Licensed Practical Nurse (LPN) license.

Schedule

• Full-Time
• $30.00/hour
• Position is with the Iowa Cross ACT program in Knoxville, Iowa

Physical Requirements

• Ability to perform essential job functions with or without reasonable accommodation.
• Position is classified at a medium physical demand level , which may include occasionally exerting up to 50 pounds of force, frequently exerting up to 20 pounds, and/or regularly exerting up to 10 pounds to move objects.
