What You’ll Be Doing Provide advanced wound care assessments, treatment planning, and ongoing management for complex wounds. Conduct routine rounding visits throughout senior living and long‑term care communities. Collaborate with physicians, nursing teams, therapists, caregivers, and community leadership. Help improve healing outcomes while reducing avoidable hospitalizations and complications. Educate staff, patients, and families on treatment plans and best practices. Support continuity of care across multiple settings and levels of service. Qualifications Active Minnesota Certified Nurse Practitioner (CNP) license. Experience in wound care, geriatrics, primary care, post‑acute care, skilled nursing, or related settings preferred. Strong clinical judgment and confidence managing medically complex patients.
