Licensed Practical Nurse (LPN) - Case Management

LPN Case Manager

Schedule: Full Time, MondayFriday, 8:00 a.m.4:30 p.m.

Assignment Details: Guaranteed Hours: 40 per week. This is a mobile position requiring travel for home visits, hospital visits, and patient office visits.

Requirements:

• Active LPN license
• 1 year of experience in Home Health or Mobile Case Management
• BLS certification required
• Access to reliable transportation
• Experience with chronic conditions such as CHF, COPD, and complex comorbidities preferred

Responsibilities:

• Support the Transitions of Care program focusing on reducing 30-day readmission rates
• Work under the direction of the RN Care Manager to assist with care plan implementation
• Schedule appointments and monitor biometric data
• Provide and reinforce patient education
• Attend patient visits and complete home visits as necessary
• Connect patients with community resources and maintain frequent patient contact
• Document interactions and updates in the EHR
• Participate in multidisciplinary care team rounds
• Manage a rolling caseload of approximately 2530 patients
