Our Lady of Consolation is committed to improving our patients' quality of life & autonomy through innovative rehabilitative services, and an exceptional level of client-centered care and attention. We are seeking a diligent and skilled Occupational Therapist to join our team in West Islip.
Job responsibilities include:
- Provide occupational therapy to residents
- Develop and improve the skills needed for the daily functioning of residents
- Accurately completes and documents all treatment procedures and functional mobility tasks in a timely manner
- Establish a plan of care in accordance with patients’ needs and capabilities
- Apply interventions to maximize performance for daily living and instrumental activities
- Prepare documentation, progress notes, reports and discharge summaries as required
Qualifications Include:
- Valid NY OT License and Credentials
- SNF and Sub-Acute/Long-Term Care experience preferred
