RN Nurse Case Manager

Providing ongoing telephonic and virtual support, the part-time RN Nurse Case Manager will engage with patients prescribed complex neurological therapies, delivering personalized education and fostering relationships to enhance their therapy journey. Key responsibilities Provide telephonic and omnichannel support through inbound and outbound calls, emails, chats, and texts Educate patients and caregivers about treatment options while collaborating with cross-functional stakeholders to ensure continuity of care Identify barriers to therapy adherence and utilize motivational interviewing techniques to support patient confidence and engagement Required qualifications Associate's degree in nursing with experience in patient education Minimum of 2 years of experience in a call center environment, preferably with a pharmaceutical company Current, unrestricted RN or NP license Background in neurological conditions and/or rare diseases Demonstrated ability to collaborate with cross-functional partners to enhance patient experience
