Outpatient Physical Therapist - 5x8.0 Schedule

Job Description

We are seeking an Outpatient Physical Therapist to evaluate and treat patients in a dynamic outpatient setting. This role involves creating and modifying treatment plans in line with patient needs and physician direction. Responsible for comprehensive patient evaluations, documentation, and treatment implementation, the physical therapist will ensure functional treatment programs that facilitate a successful return to community activities.

Responsibilities

• Complete comprehensive evaluations of assigned patients.
• Develop appropriate treatment plans and goals aimed at progressing patients toward discharge.
• Document care in a clear, concise, and timely manner, adhering to clinic policies and guidelines.
• Provide supervision and education to patients, families, and students.
• Maintain compliance with state practice acts and professional conduct codes.
• Participate in performance improvement initiatives and professional development activities.

Required Experience / Certifications / Licensure

• Degree in Physical Therapy from an accredited school.
• 6 months to 1 year of outpatient clinical experience.
• Current CPR certification.
• Active Physical Therapist license in good standing.

Schedule

This position operates on a 5x8.0 schedule with 60-minute evaluations and follow-ups every 30 minutes. It is a 13-week contract position.
