Physical Therapist

Role Overview: A licensed Physical Therapist providing personalized, one-on-one care to older adults in private residences and Senior Living Communities. The position emphasizes autonomy, allowing therapists to create and manage their treatment schedules, incorporate telehealth, and build lasting patient relationships. Key Responsibilities Perform evaluations, develop individualized treatment plans, and deliver evidence-based therapies to support mobility, independence, and wellness. Document treatment progress accurately within the EMR system, adhering to regulatory standards. Collaborate with interdisciplinary teams including PTs, OTs, SLPs, and Fitness Specialists. Educate patients, caregivers, and staff to promote safety and aging in place. Utilize telehealth and virtual care to extend access and flexibility. Qualifications & Skills Valid Physical Therapy license and degree from an accredited program. Proficiency in basic computer and EMR systems. Current CPR certification. Physical ability to perform tasks such as lifting, repositioning, and assisting with mobility, including driving to patient locations.
