Job Description

We are seeking a dedicated Physical Therapist with a minimum of 2 years of experience in home health. The ideal candidate will have experience in start of care and revisits, serving various communities.

Responsibilities

• Conduct initial evaluations, develop treatment plans, and provide therapy interventions.
• Perform start of care and revisit assessments for home health patients.
• Document patient progress and adjust care plans as necessary.
• Collaborate with other healthcare professionals to provide comprehensive patient care.

Required Experience / Certifications / Licensure

• Active Physical Therapist license in Pennsylvania.
• Minimum of 2 years of experience in home health settings.
• Proven experience with start of care and revisit procedures.

Assignment Duration

This is a 13-week contract position.
