• Coordinate and monitor Individualized Service Plan (ISP) activities and documentation requirements
• Track ISP deadlines, alerts, and compliance measures within HCSIS
• Schedule and facilitate ISP meetings with individuals served, families, staff, and stakeholders
• Support teams in developing meaningful and measurable person-centered goals
• Assist with data collection and outcome tracking related to ISP objectives
• Review progress toward goals and support plan revisions as needed
• Conduct documentation reviews and support quality assurance initiatives
• Provide guidance to staff and managers regarding ISP requirements and compliance standards
• Conduct satisfaction surveys with individuals, family members, and guardians
• Prepare reports identifying trends, compliance concerns, and opportunities for improvement
• Assist with staff training related to ISP processes, regulatory requirements, and best practices
• Participate in Positive Behavioral Supports (PBS) initiatives and related committees Who We're Looking For You may be a great fit if you are:

• Bachelor's Degree
• Minimum two (2) years of experience working with individuals with intellectual and developmental disabilities (IDD)
• Knowledge of DDS ISP systems, processes, and regulatory requirements
• Proficiency in Microsoft Office Suite and related computer applications
• Valid driver's license with the ability to travel as needed
• Strong organizational, planning, and time management skills
• Excellent verbal and written communication skills Preferred
• Experience facilitating person-centered planning meetings
• Experience with quality assurance, compliance, or documentation review processes
• Familiarity with Positive Behavioral Supports (PBS) principles Schedule
• Full-Time
• Primarily office-based position
• Occasional travel to program locations and meetings
• Flexible scheduling as needed to support ISP meetings and stakeholder participation Physical Requirements
• Primarily seated administrative and computer-based work
• Ability to exert up to 10 pounds of force occasionally
• Ability to travel to community, residential, and program settings as needed
