The Nurse Practitioner (NP) will work closely with physicians, nursing staff, and other healthcare professionals to assess, diagnose, and treat patients while ensuring continuity of care. Duties and Responsibilities

• Conduct comprehensive patient assessments and develop individualized treatment plans

• Diagnose and manage acute and chronic conditions in coordination with physicians

• Educate patients, families, and facility staff on disease management and preventive care

• Maintain accurate and timely documentation in compliance with regulatory guidelines

• Collaborate with interdisciplinary teams to ensure quality patient outcomes
