Job Details

Position: Licensed Practical Nurse (LPN)
Specialty: Long-Term Care / Post-Acute Skilled Nursing Rehab
Employment Type: Travel LPN Assignment
Location: Middlebury, Vermont
Shift: 12-hour day shifts
Schedule: Approximately 36 hours per week; schedule varies and may include every other weekend
Duration: 5 weeks
Weekly Estimated Pay: $1,680 - $1,770
Setting: Post-acute care within a skilled nursing rehab environment
Patient Population: Adults age 60+ in skilled nursing and rehabilitation care
Float Requirement: Yes, occasional/as needed floating
Documentation System: EPIC

Job Requirements

Active Vermont LPN license required at time of submission
Minimum of 1 year of clinical nursing experience required
Prior travel nursing experience required
Recent EPIC experience within the last 2 years required
BLS certification required
IV certification preferred
Dementia-capable care certification preferred
Must be available to work holidays as needed
Reliable transportation strongly recommended due to limited public transportation access in the area

Responsibilities

Provide compassionate, high-quality nursing care to post-acute and long-term care patients
Support a patient-focused rehab environment with a strong focus on safety, comfort, and recovery
Perform common unit procedures including phlebotomy, EKGs, transports, nebulizer-to-inhaler transitions, and starting IVs
Assist with patient monitoring and routine nursing tasks in collaboration with the care team
Float to long-term care and memory care units as needed
Maintain accurate and timely documentation in EPIC
Adapt to a variable schedule while helping meet the needs of the unit and patients
