Job Summary:

The Home Health LPN position offers an opportunity for licensed practical nurses to deliver patient-centered care in a supportive and dynamic environment. Focused on supporting adult patients with various complex conditions, the role emphasizes holistic assessments and comprehensive patient education.

Responsibilities:

• 
  Conduct focused assessments within the scope of practice to evaluate patient needs and progress.
  
  
• 
  Administer basic and complex wound care, ostomy care, and perform patient education on medical conditions.
  
  
• 
  Collaborate with the care team to develop and update individualized care plans based on patient evaluations.
  
  
• 
  Provide emotional support and build strong connections through 45-60 minute patient visits.
  
  
• 
  Manage a caseload of 5-6 patients daily, adjusting for travel times and clinical complexity.
  
  
• 
  Document patient interactions and clinical observations accurately in compliance with agency protocols.
  
  

Qualifications:

• 
  Possess a valid Washington state license or have the application process in progress.
  
  
• 
  Have at least 2 years of recent experience in Home Health and Hospice care.
  
  
• 
  Demonstrate proficiency in wound care, ostomy management, and patient education.
  
  
• 
  Hold current BLS certification.
  
  
• 
  Be willing to travel up to 100 miles daily and manage a flexible visit schedule.
  
  
• 
  Must submit two recent professional references, including at least one supervisor.
