LPN (Home Health)-{169879} Location: Pensacola, FL 32514 · Coverage Area: Escambia, Santa Rosa, Okaloosa, Walton Counties (expected to work in 2 counties : Escambia/Santa Rosa or Okaloosa/Walton) Pay: $37.30/hr. · Mileage Reimbursement: $0.49/mile Schedule: Monday to Friday, 8:00am-5:00pm, 40 hours/wk. On-Call: 1 day per week Overview We’re hiring an LPN to provide in-home nursing care while maintaining each client’s dignity, safety, and independence. This is an onsite, field-based home health role supporting patients across assigned counties in the Pensacola area. Key Responsibilities Provide skilled nursing care per the client’s plan of care and physician orders Monitor vital signs and report abnormal findings promptly Administer medications and treatments as ordered (including oxygen support, feedings, ostomy care when applicable) Perform wound care, assessments, and routine skilled nursing visits Coordinate and follow care routines established by PT, OT, and Speech Therapy Maintain infection control practices and report potential infections/communicable diseases Document all care provided, patient progress, and outcomes accurately and timely Order supplies/medications as needed to ensure appropriate stock on hand Monitor incidents, assess the patient after incidents, and complete required incident reporting Required Skills & Qualifications Active LPN license (required) Home Health nursing skills including wound care and patient assessment Experience with HCHB and OASIS expected for home health documentation Keywords LPN, Licensed Practical Nurse, Home Health, OASIS, HCHB, Wound Care, Patient Assessment, Documentation, Infection Control, Field Nurse, In-Home Care
