Duties

Duties and responsibilities include, but are not limited to the following:

• Facilitates access into Outpatient Mental Health (OPMH) ensuring timely access to care which may include intakes for medication management and psychotherapy, transfers of care, and triaging needs for other services.
• Conducts consult management including triage and disposition, utilizing CPRS, Consult Tracking Management, and other applicable computer applications and/or dashboards. Coordinates appointments which may include scheduling.
• Collaborates frequently with Referral Coordination Team (RCT), Care in the Community (CITC) and Health Administrative Services (HAS) to streamline assessment for drive-time and wait time eligibility, community referrals, and VA scheduling.
• Collaborates with various OPMH programs/teams, including CBOCS, and the VA facility at large to facilitate access to clinically appropriate OPMH services.
• Liaises with Inpatient Psychiatric Unit and Psychiatry Consult and Liaison Service to ensure seamless transition from inpatient to outpatient care.
• Provides regular consult status reports to MH Executive Leadership via supervisor.
• Participates in relevant meetings and workgroups as it relates to consult management.
• Facilitates outpatient psychosocial/psychoeducational groups for Veterans.
• Provides coverage for OPMH RN case managers and other RN roles in OPMH as needed which may include medication administration, care coordination.
