What You’ll Do
Perform initial and ongoing client assessments.
Develop and update individualized care plans.
Provide clinical oversight and guidance to care teams.
Communicate effectively with clients, families, and healthcare professionals.
Ensure compliance with state regulations and agency standards.
Routine Med Set ups for clients.

Who You Are
Licensed RN in Colorado
2+ years of nursing experience (home health or hospice preferred)
Current CPR certification
Strong assessment and communication skills
Valid driver’s license and reliable transportation
Ability to work independently and manage multiple priorities
