This role involves providing patient-centered care either in home settings or hospice facilities, ensuring comfort, dignity, and quality of life for patients and their families throughout the hospice journey.Key ResponsibilitiesPerform comprehensive assessments addressing physical, emotional, and spiritual needs of patients.Conduct initial physical evaluations and develop individualized care plans collaboratively with the healthcare team.Administer medications and manage symptoms following established protocols.Educate and support patients and families about end-of-life care options and resources.Coordinate services and maintain accurate documentation of patient care activities.Communicate changes in patient condition to the interdisciplinary group and keep primary and consulting providers informed.Perform venipuncture or collect lab specimens as ordered.Education and ExperienceGraduation from an accredited nursing program with current Registered Nurse licensure valid for practice.Previous experience in hospice, palliative, or critical care settings is preferred.Completion of hospice philosophy and care training is required.RequirementsCurrent Basic Life Support (BLS) certification according to American Heart Association standards.Completion of Mandatory Reporter training within six months of hire.Strong oral and written communication skills.Basic computer proficiency and ability to perform simple mathematical calculations.Compliance with all federal, state, and professional licensure and regulatory training requirements.This position is based in a regional healthcare setting within the Midwest, offering a warm and collaborative environment dedicated to compassionate end-of-life care.
